"Home improvements" Grant Scheme Application Form

Home Improvement Scheme Lady and Flower

 

The Mental Health Foundation has announced a new grant-making scheme to fund new and innovative projects for people with Alzheimer’s disease and other dementias living in care homes.

 

Funding of between £5,000 - £25,000 will be provided to selected projects which can run for up to two years. The Foundation will provide some additional support to projects and evaluate the projects.

  

The scheme is being funded out of a substantial legacy left to the Foundation to do work in connection with Alzheimer’s disease.

Stage 1 – Eligibility Quiz/Pre-Application Screening

 

These questions are to find out whether you are eligible to apply for a grant. To pass through this section and get to the full application form you must answer all 5 questions correctly.  Incorrect answers mean you’re not eligible to apply for a grant. 

 

If a question is answered incorrectly you will be taken to a screen that gives a brief explanation as to why you have failed to meet the essential selection criteria. It is advisable to read through the frequently asked questions on this website before answering these questions.

 

Stage 2 – Application Form

 

This is the application form which is in 5 sections. The first section is about the person who is making the application. If you are the named person in this section you will be the point of contact for the grant and all letters will be sent to you. If it is a group applying it is important that this person will be in a position to receive and pass on information as necessary.

 

The other sections ask for further organisational details and details about the actual proposed project.

 

There is no word limit on each text box but please try to limit your answers to a maximum of 500 words per box.

 

All questions marked with an asterisk are mandatory. If the question does not apply to you or you do not know the answer please type ‘not applicable’ or ‘don’t know’ in the box provided otherwise the application form cannot be submitted. You will automatically be told which answers have been left blank before you submit the application.

 

The application form can be saved at any time to enable you to come back to it and finish it off.  To save it this way, go to the bottom of any page on the questionnaire and click on the tab that says ‘save and finish later’. You will then be asked to create an account which requires an email address and a password. 

 

To return to the saved application return to the Home Improvements Grant Scheme page on our website and click the link that says ‘continue saved application form’. To enter your account, enter your email address and password. Once in your account, click the ‘Home Improvements’ Application Form link.

 

Stage 3

 

This allows you to review your application before you submit it. At this point you are still able to amend the application form.

 

Stage 4

 

Submission of the application form. Once submitted, you will be unable to make any changes or corrections. You will then be asked if you would like to create an account. Although this isn’t mandatory, it is advised. Creating an account generates a short email message from the Foundation letting you know we have received your application. 

 

If you don’t create an account, you will receive you confirmation email once we have checked your application form.
